A C-section is major abdominal surgery — and recovery from it is often more complex than new parents expect. The swelling, the heaviness in your abdomen and legs, the tenderness around your incision, the feeling that your body is holding on to fluid it cannot seem to move — these are not just the normal discomforts of the postpartum period. They are the signs of a lymphatic system that has been disrupted by surgery and needs support.

Manual lymphatic drainage (MLD) is one of the most effective evidence-informed tools available for post-surgical recovery. How MLD Supports C-Section Recovery

When you have a C-section, the surgical incision disrupts the superficial lymphatic vessels in the lower abdomen — the same vessels responsible for draining fluid from your pelvic region and lower body. At the same time, your body is managing the normal hormonal fluid shifts of the postpartum period. The result is often significant edema in the abdomen, legs, ankles, and feet that can persist for weeks or longer without support.

Manual lymphatic drainage works with the superficial lymphatic system through slow, gentle, rhythmic movements that stimulate lymph flow without placing any direct pressure on your healing incision. Sessions are specifically designed around post-surgical anatomy, working proximal to the incision to open drainage pathways before addressing the areas closer to the scar.

When to Start MLD After a C-Section

This is one of the most common questions we hear, and the honest answer is: it depends on your individual recovery, and you should always get clearance from your OB, midwife, or care provider before starting any postpartum bodywork.

4–6 weeks postpartum is typically when clients begin MLD after a C-section. By this point, the incision has usually closed and initial healing is underway, and most clients have had their six-week OB visit.

Some clients begin as early as 2–3 weeks with gentle, proximal-only work that stays well clear of the incision and focuses on the upper body and axillary lymph nodes to open drainage pathways from a distance. This is always assessed on an individual basis.

What a Session Looks Like

MLD sessions are very different from a standard massage. The pressure is intentionally light — far lighter than you might expect from a therapeutic treatment. This is not a relaxation massage; it is a precise, clinically-trained technique that works with the superficial lymphatic system just beneath the skin.

You will be positioned comfortably on the table — typically on your back or side — with full bolster support. Your certified manual lymphatic drainage therapist will work systematically, beginning at the lymph node clusters furthest from your incision and gradually working toward the area of surgical disruption. The incision itself is not directly worked until it is fully healed and closed, and only with your OB's clearance.

Sessions are available in 60, 75, and 90-minute lengths. For post-C-section recovery, most clients find 75–90 minutes most effective, particularly in the early weeks. Most clients see meaningful improvement in swelling within 3–6 sessions in the first 6–8 weeks postpartum.

Kiera Nagle, MA, LMT, CMLDT, CLC has been practicing massage therapy for 18+ years. She holds the Certified Manual Lymphatic Drainage Therapist credential (Vodder technique) and has 17 years of clinical experience in perinatal and postpartum bodywork. For 10 years, she served as a clinical supervisor at Northwell Katz Women's Hospital, and spent 12 years on faculty at Pacific College of Health and Science. She is also a published contributor to Massage Magazine and a Massage Therapy Foundation grant recipient. She is the owner / founder and lead therapist at MaMassage Brooklyn. She does all the hiring and training of the other LMT’s and CMLDT’s herself- and they all provide similarly high quality care.

Frequently Asked Questions

Is lymphatic drainage safe after a C-section?

Yes — when performed by a trained, certified MLD therapist who understands post-surgical anatomy and timing. MLD is specifically indicated for post-surgical edema and scar support. The key is working with a therapist who knows how to adapt the technique to a healing incision, and ensuring your care provider has cleared you for postpartum bodywork.

Will it hurt?

MLD is one of the gentlest forms of therapeutic bodywork. The pressure used is very light — roughly the pressure of a coin on your skin. Many clients find it deeply relaxing. If anything feels uncomfortable at any point, we adjust immediately.

Can I do MLD while breastfeeding or chestfeeding?

Yes. MLD is fully compatible with breastfeeding and chestfeeding. There is no contraindication.

Do I need a doctor's referral?

You do not need a formal referral, but we ask that you have been cleared by your OB or midwife for postpartum bodywork before your first session. If you are unsure, call your care provider and ask — most providers are familiar with postpartum MLD.

How many sessions will I need?

Most clients see meaningful improvement within 3–6 sessions in the first 6–8 weeks postpartum. Some clients continue monthly sessions throughout the first year to support scar tissue remodeling and lymphatic health. Every recovery is different — we will give you an honest assessment at your first session.
